COURSE REQUIREMENTS

Players must bring a packed lunch & plenty of drinks (no food/drink available on site).Shin-pads and clothing appropriate to the weather (waterproofs, sweaters etc.)  Sun-cream (if needed) TRAINERS ONLY at all venues.

When we receive your form you will be booked on the course but not contacted!

Emergency Treatment -In the event of any emergency, medical treatment may be authorised on your behalf by Shay Stadium Community Football staff and their representatives.

Additional Venues - On rare occasions we use alternative venues and children will be escorted to these areas on foot by our staff.

Photographs - Shay Stadium Community Football may want to take photographs that include your child for our promotional purposes. The photographs may be used on leaflets, booklets and display panels. They may also be placed on our website on the internet. By completing the form (below) you allow us to use these photographs on leaflets, booklets and display panels. Please inform us (in the 'notes' section) if you do not wish your child to be photographed.
